Registration Form We ask that
you pre-register for GLYC 2010 by Friday, April 9. This will be
very helpful to us in planning for the best possible day for your
teens. Check back soon for downloadable registration forms. On the day of the conference, registration will
take place from 9:00-9:30am and walk-ins will be accepted.
You can fax or mail your registration.
If you fax the registration, please indicate that you are mailing in the payment.

Permission Form
Signed permission forms are required for each teen! Please
note that multiple teens from the same family still need individual
permission forms. Check back soon for downloadable permission forms. You do not need to
send in the permission forms with your registration, simply bring them with you
to the conference.
